Predeceased by parents Lawrence & Ruth Phalen. Survived by his loving wife Jan; children John Phalen, Robert (Claudia) Phalen, Kelly Phalen; grandchildren Marco & Laila Phalen; sister Sue (Steve) Waffle; brother Jim Phalen; nieces, nephews & cousins.

Jack was active in Cub Scouts, Gates-Chili Little League and Section V Wrestling.

Family & friends may call Saturday, March 7, from 12 - 2 PM at James R. Gray Funeral Home, 1530 Buffalo Rd. Services and interment will be private. A gathering will follow visitation, the details of which will be announced on Saturday.
